SHINING LIGHT FOUNDATION

 

MINUTES OF MEETING HELD: Friday, September 11, 2009 at 4:30 pm

 

LOCATION:  UBS Financial Services, 1021 E. St. Mary Blvd., Lafayette, LA

Special meeting called for the purpose of considering requests for musical instruments recently received by John.  Further, meeting for the purpose of compiling documents required for submitting applications for grants to Shining Light Foundation from Super 1 Foods and from Junior League of Lafayette

 

The following donations were received today and will be deposited in our account on Monday:
 Janice and George Roane in honor of Patrick Roane $30.00;
 Wal-Mart Foundation $1,000.

 

New Applications:
Request from J. Wallace James Elementary School:
 Dontravia Hamilton, trombone $200, book $8, Cleaning kit $15 = $223 less
    Parent contribution $55.75        = $167.25

 

Request from Acadian Middle School:
 Ethan Rabeaux, saxaphone $217.50 less Parent contribution $72.50 = $145.00

 

Requests from Plantation Elementary School:
 Donovan Wright, trombone $200 less Parent contribution $50 = $150
 Kelly James Matthews, Jr., flute $200 less Parent contribution $50   = $150
 Hannah Zavala, clarinet $200, less Parent contribution $50               = $150
 D’vonte Wright, tuba $360 less Parent contribution $90                    =$270

 

On motion of Mike, seconded by John, requests approved on condition that Michelle will contact Prof Erny’s to find out if the elementary schools use tubas in their bands.

 

Discussion was held regarding the requirement by the IRS for non-profit organizations to file a Form 990-N.  Michelle will ask Bruce if we filed such a form.

 

Michelle will add a request on all application forms for the teacher’s e-mail address.
